In this fully remote role, youll be a vital lifelineconnecting with patients, guiding them in managing their conditions, and making a real impact on their health outcomes.

What Youll Do

Connect & Care : Make monthly outreach calls to patients enrolled in our CCM program.

Personalized Care Plans : Collaborate with patients and providers to design and update individualized care strategies.

️ Early Intervention : Monitor symptoms, identify risks, and escalate care before complications arise.

Coordinate Care : Work seamlessly with primary care providers, specialists, and community resources.

Patient Education : Equip patients and caregivers with knowledge on disease management, medication adherence, and healthy habits.

Document with Precision : Keep accurate and timely patient records in our EHR system.

Stay Compliant : Follow CMS guidelines for CCM while promoting preventive care and lifestyle improvements.

What You Bring

• Valid RN license in the Philippines.
• Minimum 1 year of clinical nursing experience (chronic care, case management, or primary care experience is a plus).
• Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure.
• Excellent communication, critical thinking, and organizational skills.
• Comfortable with EHR systems and virtual care tools.
• Able to work independently while thriving in a collaborative team setting.
