Job Title : Medical Records Reviewer

Schedule : M-F; Flexible, overlap between 8am-5pm Pacific

Our Client is a leading provider of in-home and community-based rehabilitation programs for individuals with brain and spinal cord injuries in California.

JOB SUMMARY :

We are seeking a Medical Record Reviewer with a background in nursing or therapy to evaluate and analyze patient medical records. The ideal candidate will have experience in healthcare settings and be able to assess the accuracy, completeness, and compliance of medical documentation. The role requires attention to detail, critical thinking, and a solid understanding of medical terminology and standards.

DUTIES AND RESPONSIBILITIES :

Medical Record Review :

  • Review and analyze patient medical records for completeness, accuracy, and compliance with regulatory standards.
  • Ensure proper documentation of diagnoses, treatments, and patient progress.

Quality Assurance :

  • Identify discrepancies, errors, or omissions in medical records and take corrective actions as necessary.
  • Collaborate with medical staff to ensure accurate documentation and improve record-keeping practices.
  • Regulatory Compliance :

  • Ensure medical records adhere to legal, ethical, and regulatory requirements.
  • Keep up-to-date with industry standards, laws, and regulations related to medical documentation.
  • Collaboration with Healthcare Providers :

  • Work closely with physicians, therapists, and other healthcare professionals to clarify and ensure accurate patient records.
  • Report Generation :

  • Prepare reports on medical record compliance, identifying trends, discrepancies, or areas of improvement.
  • Provide recommendations to improve documentation practices based on findings.
  • Requirements

  • At least 3 years of experience as a nurse, therapist, or in a healthcare-related role is preferred.
  • Familiarity with medical terminology and documentation standards.
  • Proficient in using electronic medical records (EMR) systems and other healthcare software.
  • High attention to detail and accuracy in medical record review.
  • Strong analytical and problem-solving skills.
  • Excellent communication skills, both written and verbal.
  • Ability to work independently and manage multiple tasks.
