Job Overview :
E-TeleConnect is looking for a Registered Nurse (Spanish-Speaking) Case Management & Care Coordination to join our growing healthcare BPO team. We are looking for a Registered Nurse (RN) licensed in the Philippines with strong clinical judgment, conversational Spanish, and a working understanding of the U.S. healthcare and home health system. This position focuses on case management and care coordination for patients under home health, post-acute, or chronic care programs in the U.S. The RN Care Coordinator will serve as a key liaison between patients, caregivers, healthcare providers, and payersensuring timely care delivery, accurate documentation, and adherence to plan-of-care protocols.
Key Responsibilities
- Coordinate patient care plans and support case management activities under the supervision of U.S.-based Case Managers.
- Conduct telephonic outreach to patients and families (in English and Spanish) to gather updates on clinical status, confirm visits, and monitor adherence to care plans.
- Manage and update patient records in Electronic Health Record (EHR) systems, ensuring all documentation meets U.S. home health compliance standards.
- Assist in care transitions by confirming hospital discharges, scheduling follow-up visits, and ensuring continuity of care between providers.
- Identify and escalate clinical or social risk factors that require urgent intervention by clinical leads or physicians.
- Facilitate communication among multidisciplinary teams (physicians, physical therapists, social workers, and field nurses).
- Review and reconcile documentation such as orders, visit notes, and OASIS data for accuracy and completeness.
- Maintain a high level of patient confidentiality and compliance with HIPAA and organizational standards.
- Participate in regular team huddles, case reviews, and ongoing training related to U.S. home health and chronic care management.
Qualifications
Active Registered Nurse (RN) License in the Philippines (in good standing).Minimum of 3 years of clinical experience in hospital, community health, or home health settings.Conversational Spanish proficiency (able to conduct basic patient communication and coordination in Spanish).Fluent in English, with strong written and verbal communication skills.Working knowledge of U.S. healthcare workflows, home health operations, and case management processes.Familiarity with EHR systems (e.g., WellSky, Alora, Homecare Homebase, or similar platforms).Strong organization, prioritization, and multitasking skills.Proficient in Microsoft Office / Google Workspace and virtual communication tools (Zoom, Teams, RingCentral, etc.)Ability to multitask and manage multiple cases under time-sensitive conditionsFluency in both English and Spanish is essential, as you'll interact with Spanish-speaking patients and healthcare professionals.
