Transition Manager - Change and Release Management.

Ensure Application Development incorporates supportability, observability, and maintainability practices e.g., logging, monitoring, coding standards.

Review solution designs and delivery plans for operational readiness.

Define transition scope, timelines, milestones, and acceptance criteria.

Facilitate cross-team coordination such as Application Development, Operations, QA, business stakeholders.

Conduct transition readiness assessments prior to Go-Live.

Oversee the execution of the handover process between Application Development and Operations or Support.

Track and report transition status, risks, and issues to leadership.

Capture lessons learned and ensure improvements are applied to future transitions.
