As a USRN – LTAC / Rehab Nurse, you will provide skilled, patient-centered care to medically complex individuals in a Long-Term Acute Care Hospital (LTAC) or Inpatient Rehabilitation Facility (IRF). You will play a vital role in helping patients recover from serious illnesses, surgeries, or injuries that require extended hospital-level care.
Key Responsibilities :
Deliver direct nursing care to patients requiring long-term acute care, including ventilator support, complex wound care, and IV therapy.
Monitor and assess patients’ conditions, manage symptoms, and report changes in status to the care team.
Collaborate with multidisciplinary teams including physicians, physical therapists, respiratory therapists, and case managers to create and implement care plans.
Educate patients and families on recovery goals, discharge planning, and health maintenance.
Accurately document all care activities in the electronic medical record (EMR) in compliance with facility policies and regulatory standards.
Ensure strict adherence to infection control practices, medication safety protocols, and patient confidentiality (HIPAA).
Qualifications :
Active USRN license (state-specific or compact license as applicable).
Minimum 1 year of bedside experience in acute care, LTAC, or rehabilitation settings preferred.
BLS and ACLS certifications required.
Strong clinical judgment, communication, and critical thinking skills.
Work Setting :
Inpatient LTAC hospital or rehabilitation facility
Shift-based work (days, nights, weekends, or rotating shifts)
