What you will be doing :

• Conduct Intake Interview and screening
• Make follow-up Health Consultations
• Health Counseling and Education
• Provide recommendation of lifestyle changes to patients / clients
• Refer clients / members to healthcare providers

What you need to have :

• College graduate of Social Work
• Minimum of 12 months experience in Direct Patient Care such as :
• Long-Term Care
• Home / Community-Based Services
• Hospital / Facility Settings
• Case Management & Rehabilitations
• Experience working with managed care organizations, including Medicare and / or Medicaid recipients, (HMO's)
• Experience with Healthcare BPO (voice) member-calling experience

What you will earn (Up to more than P30,000 Monthly) :

• P26,000 Basic
• P1,000 Flex Fund Allowance
• 20% Night-differential
• Paid Trainings and Leaves with conversion
• HMO on Day 1 with 1 additional dependent
• 13th Month pay
• Annual Appraisals based on performance
