This role is responsible for delivering quality health care service within the provision of safe, effective, and quality pharmaceutical products and pharmaceutical care through pharmacologic and non-pharmacologic strategies to prevent adverse drug events and to optimize individualized therapeutic outcomes.This role reports directly to the Pharmacy department.What's The Role All About?Job responsibilities include, but not limited to : Reading, validating, and interpreting prescriptions.Collecting relevant patient-specific information for an effective review processPerforming medication reconciliation by reviewing patient's current medication in comparison to the new drug orders.Reviewing patient's drug orders or prescription for appropriateness prior to dispensing includes evaluation of the appropriateness of drug, dose, frequency, and route of administration, therapeutic duplication, real or potential allergies or sensitiveness, real or potential interactions between medications and other medication or food, variation from hospital criteria for use, patient's weight and other physiological information and other contraindications.Designing a comprehensive drug therapy plan for patient-specific problemsWho Are We Looking For?An effective Clinical Pharmacist must possess the following skills : Detail-oriented : Real-time documentation of all identified drug related problems and recommendations in Pharmacists' Intervention Form and Hospital Information System (HIS) Electronic Medical RecordHealth education by communicating all identified drug therapy problems to doctors, nurses, and other healthcare professionals and providing drug information to doctors, nurses, and other healthcare professionals.Interpersonal skills : Providing medication counseling and education to patient / s when necessary and answering drug inquiries and concerns in a timely and efficient manner.Ideally, a candidate must be a graduate of BS Pharmacy with required Major in Clinical Pharmacy and / or with Doctor of Pharmacy degree (PharmD), and with PRC License (RPh), preferably with work experience.
