Care Coordinator Nurse

Visaya KPO is looking for a Care Coordinator Nurse with strong clinical experience to join our healthcare team! If you have a heart for patient care and experience in hospital or case management, we want you on board.

What you’ll do :

• Manage and coordinate chronic care plans
• Monitor and assess patient conditions
• Work with providers for seamless care transitions
• Prepare reports and patient documentation
• Educate and support patients & families

Qualifications :

BS Nursing graduate with active PHRN license

2–3 years hospital / clinical experience

Case Management or Transition of Care experience preferred

BPO experience, a plus

Willing to work onsite in Bacolod (Night Shift)
