In this fully remote role, you’ll be a vital lifeline—connecting with patients, guiding them in managing their conditions, and making a real impact on their health outcomes.

What You’ll Do

• Connect & Care: Make monthly outreach calls to patients enrolled in our CCM program.
• Personalized Care Plans: Collaborate with patients and providers to design and update individualized care strategies.
• Early Intervention: Monitor symptoms, identify risks, and escalate care before complications arise.
• Coordinate Care: Work seamlessly with primary care providers, specialists, and community resources.
• Patient Education: Equip patients and caregivers with knowledge on disease management, medication adherence, and healthy habits.
• Document with Precision: Maintain accurate and timely patient records in our EHR system.
• Stay Compliant: Follow CMS guidelines for CCM while promoting preventive care and lifestyle improvements.

What You Bring

• Valid RN license in the Philippines
• Minimum of 1 year of clinical nursing experience (chronic care, case management, or primary care experience is a plus)
• Strong knowledge of chronic conditions such as diabetes, COPD, hypertension, and heart failure
• Excellent communication, critical thinking, and organizational skills
• Comfort using EHR systems and virtual care tools
• Ability to work independently while thriving in a collaborative team environment

Bonus Points If You Have

• Familiarity with Medicare Chronic Care Management documentation and compliance
• Experience with motivational interviewing or other patient engagement techniques
