Position Overview
The Afterhours Intake supports the intake and referral processing operations for Supportive Home Health. This role is responsible for managing incoming referrals, reviewing documentation, coordinating patient onboarding, and ensuring timely transition of care from hospital to home health services.
This is a non-bedside, operations-based clinical role focused on documentation management, intake coordination, and initial patient contact.
Key Responsibilities
1. Intake & Documentation Management
- Receive and manage incoming referrals via fax, email, and chat platforms
- Review referral packets for completeness including H&P, orders, discharge notes, authorizations, and supporting clinical documents
- Organize and upload all documentation into the EMR system
- Ensure documentation is complete and ready for Case Management endorsement
2. Clinical Review & Intake Triage
- Conduct initial clinical review of referral documentation to determine eligibility for home health services
- Identify missing documentation or inconsistencies and escalate accordingly
- Apply basic clinical judgment to determine if the patient is appropriate for home health admission
3. Patient & Care Coordination
- Conduct initial outreach to patients to:
o Confirm referral to Supportive Home Health
o Provide overview of services (RN, LVN, PT, OT, ST as stated on physician orders)
o Set expectations for follow-up calls and home visits
- Coordinate handoff to Case Management and Scheduling teams
4. Data Entry & EMR Management
- Accurately input patient demographic and clinical data into EMR/EHR systems
- Track referral status and pending documentation
- Maintain timely updates on intake progress and readiness for admission
Required Qualifications
Experience Requirements
Minimum 2 years of combined experience in one or more of the following:
- Case Management
- Utilization Management
- Intake / Referral Coordination
- Prior Authorization
- Denials and Appeals
- Healthcare Operations / Healthcare BPO
- Care Coordination
- Medicare / Medicaid (Medi-Cal)
Experience utilizing Electronic Medical Record (EMR/EHR) systems such as:
- Home Health MD (HHMD)
- Axxess
- Kinnser / WellSky
- or similar healthcare documentation platforms

Preferred (Not Required)
- Home Health or post-acute care experience
- Intake or referral coordination experience
- Familiarity with Medicare and Medi-Cal home health requirements
- Experience working with multidisciplinary healthcare teams Scope
- No direct patient care responsibilities
- Responsible for intake processing, referral coordination, documentation review, and EMR data management
- Works closely with Case Management, Scheduling, Quality Assurance, and other Clinical Operations teams to ensure timely and accurate patient onboarding
- Escalates documentation deficiencies and workflow issues to the appropriate clinical staff when necessary
