Position: Healthcare Associate – Clinical Tele Triage

Role Overview

We are looking for a Healthcare Associate – Clinical Tele Triage to provide remote patient assessment, symptom evaluation, and care guidance through telehealth platforms. The ideal candidate will have strong clinical assessment and critical-thinking skills, excellent communication abilities, and experience working with telehealth and Electronic Health Record (EHR) systems.

The role involves conducting initial patient assessments, determining the appropriate level of care, escalating urgent or critical cases, and ensuring accurate documentation in accordance with established clinical protocols.

Key Responsibilities

• Assess patient symptoms and medical concerns over the phone using established clinical tele-triage protocols.
• Conduct initial triage assessments through telehealth platforms.
• Evaluate patient conditions and determine the appropriate level of care, including:
• Emergency Room (ER)
• Urgent Care
• Primary Care
• Self-care/home management, where appropriate
• Provide patients with appropriate health education regarding symptom management, preventive care, and follow-up.
• Identify and promptly escalate critical or potentially life-threatening conditions.
• Coordinate with emergency services and relevant healthcare teams when required.
• Accurately document patient interactions, assessments, recommendations, and outcomes in EHR systems.
• Relay relevant patient information to physicians, nurses, care teams, and other appropriate stakeholders.
• Assist patients with appointment scheduling and coordination of follow-up care.
• Support medication-related queries and renewal requests in accordance with established clinical protocols and scope of practice.
• Maintain patient confidentiality and comply with applicable healthcare privacy and data protection requirements.
• Follow organizational clinical guidelines, escalation procedures, and quality standards.
• Provide feedback and recommendations to improve tele-triage processes, protocols, and patient outcomes.
• Maintain a professional, empathetic, and patient-focused approach during all interactions.

Required Qualifications

• USRN (United States Registered Nurse) license is mandatory.
• Strong clinical assessment and patient-triage skills.
• Excellent critical-thinking and decision-making abilities.
• Strong verbal and written communication skills.
• Experience in telehealth, tele-triage, remote patient assessment, or similar healthcare environments.
• Hands-on experience working with Electronic Health Record (EHR/EMR) systems.
• Ability to identify urgent and emergency clinical situations and escalate appropriately.
• Strong documentation and attention to detail.
• Ability to work independently while following established clinical protocols and guidelines.
• Strong patient-service orientation with empathy and professionalism.

Preferred Qualifications

• Previous experience in clinical triage, nurse advice lines, telemedicine, urgent care, emergency care, or primary care.
• Familiarity with healthcare scheduling and patient management systems.
• Experience handling medication refill or renewal requests.
• Familiarity with U.S. healthcare processes and terminology.
• Experience working in a high-volume patient-support or contact-center environment.

Ideal Candidate

The ideal candidate is a USRN with strong clinical judgment and tele-triage experience who can confidently assess patient symptoms remotely, recognize situations requiring immediate intervention, communicate clearly with patients, and ensure appropriate care navigation while maintaining accurate clinical documentation.
