USRN - New York (Clinical Triage and Patient Support)

ROLE SUMMARY

Provides telephone-based clinical triage and patient support for a 24x7 nurse triage line.

The role independently assesses symptoms using approved protocols, identifies red flags,

determines appropriate disposition, provides patient guidance, and escalates urgent or

complex cases to physicians or emergency services. This profile directly aligns to the

OP4073 core nurse-triage requirement.

KEY RESPONSIBILITIES

• Conduct structured clinical assessment of patient symptoms, duration, severity, relevant history, and risk factors.
• Apply approved triage protocols and clinical decision-support tools to determine the appropriate level of care.
• Identify clinical red flags and promptly escalate urgent or emergent cases to the physician/on-call provider or emergency services.
• Determine disposition, including self-care guidance, non-urgent appointment scheduling, physician escalation, urgent care, or emergency department referral.
• Provide clear, evidence-based patient education and next-step guidance within approved scope and protocols.
• Document assessment, disposition, escalation, and follow-up actions accurately and in real time in the designated EMR/CRM.
• Meet defined service, quality, documentation, schedule-adherence, and HIPAA requirements in a 24x7 environment.
• Participate in training, certification, quality calibration, coaching, and continuousimprovement activities.

MINIMUM QUALIFICATIONS

• Active, unrestricted New York State RN license – mandatory
• Active Registered Nurse credential
• Prior nurse-triage experience
• Orthopedic / musculoskeletal experience preferred
• Physician-group, health-system, ambulatory, or similar clinical experience preferred
• BLS preferred; ACLS an advantage
• Experience with EMR/CRM systems; ModMed exposure an advantage
