Schedule : Mon-Fri 9am-6pm PST
Mon-Fri 10am-7pm PST
Thur-Monday 9am-6pm PST
Responsibilities
Manage patient intake and scheduling for home health and hospice visits
Maintain accurate records in EMR or scheduling software
Coordinate with internal staff for daily schedules and patient care updates
Ensure timely follow-ups and reminders with patients and caregivers
Assist with administrative tasks as needed
Requirements
Prior experience in home health administration or scheduling (required)
Excellent attention to detail and ability to manage multiple tasks
Fluent in English (written and spoken)
Familiarity with scheduling or EMR software
Proactive and able to hold teams accountable for deadlines
Scope
Full-time remote position (40+ hours / week)
Initial focus on scheduling and intake, with potential to take on more responsibilities
