An Allied Care Coach provides support and guidance to individuals within the healthcare field, often assisting with patient care coordination, health management, and navigating the healthcare system. They may work with patients, families, or other healthcare professionals to optimize health outcomes and promote well-being.
Care Coaches work with patients to improve and facilitate communication about goals, values, and preferences with their loved ones and care teams. They discuss goals of care to get a better understanding of what is most important to patients, their wishes, and priorities.

Profile 1 – Graduate of Bachelor’s Degree (with or without license)

Human Services

Experience Required :
1-year direct adult patient care such as but not limited to
Long term care
Home and community-based services
Hospital / facility settings
Case management and / or working with managed care organizations
OR
BPO healthcare – MEMBER services via phone
Not Qualified Experiences :
Catering to Providers and / or pediatrics only
Interactions via email / chat only
Any tasks not directly talking to patients
Profile 2 - Associate Graduate / College Graduate (Non-Allied Courses)
Experience Required :
